This is for a blank vintage 1960s 8 1/2" x 11" Fantasy Amateur Press Association Top Ten Egoboo Poll Award. There are areas left blank to fill in a name/title, the number the award-winner placed, and "Standings by Finishing in Individual Categories," along with a place for it to be signed by the Vice President. Very scarce and hard to find! For those who don't know, but might be curious, "Egoboo" was short for "Ego-Boost.") Sheet has minor creasing in the lower left corner area, and there is some fading to the word "PRESS" (see scan).
